Today in healthcare: Monday 30 September

This weekend, Randeep Ramesh, the Guardian's social affairs editor, wrote that the controversial cancer drugs fund, which pays for expensive treatments considered poor value for money by regulators, will get a £400m boost – effectively extending its life for two years.

The Observer reported that a private health insurance company has been forced to take down an advert from its website after it tried to sell its products by claiming that the NHS had been responsible for 13,000 needless deaths since 2005. The claim, made by Bestmedicalcover, was found by the Advertising Standards Authority (ASA) to have used an "appeal to fear to sell private health insurance and that it was not justified to do so".

The Telegraph featured a story that said NHS bureaucrats have banned the term 'elderly' from leaflets for fear of offending patients.

The paper also quoted health secretary Jeremy Hunt as saying that GPs need "rigorous inspection".

Meanwhile, the BBC said that charities have warned the government over the impact of an ageing population. And it featured a video with an expert from Cardiff University saying that combining traditional forms of Chinese and Western medicine could offer new hope for developing new treatments for some cancers.

It also reported that Professor Steve Field, the NHS's first chief inspector of GPs, warned that failing GP surgeries face closure.

The Sunday Times (subscription) ran a story that said the NHS must reform the way it treats children to prevent 2,000 youngsters dying needlessly every year due to poor care. The call comes from Hilary Cass, president of the Royal College of Paediatrics and Child Health, who said children in Britain were far more likely to die from diseases or infections such as asthma, pneumonia and meningitis than elsewhere in western Europe.

And, it said that almost half of maternity units have been forced to turn away women in labour because they are full and struggling to cope with the highest birth rate for 40 years

Why didn't the GMC prosecute Mid Staffs directors?

The GMC, on legal advice, has decided not to prosecute four medical directors, who were in place during the Mid Staffs trauma years, because of lack of evidence. The mind boggles as it has never boggled before: hundreds of needless deaths and wards full of patients afloat in their own excrement. And yet there is "insufficient evidence". The law is indeed an ass. And the GMC is toothless, impotent and living in another universe.

I have some questions for the GMC and for Tom Kark, the QC who gave the legal judgment.

• Apparently, 26 witnesses were questioned and it was their replies that offered insufficient evidence to prosecute. Who were these witnesses: clinical colleagues, hospital managers, junior doctors, or perhaps (God forbid) a few patients or relatives of the dead? Tom Kark says that "the overwhelming preponderance of the evidence is favourable to the defendant doctors". They were "well-intentioned individuals doing their best in difficult circumstances to improve poor standards". To me this smells like the cosy evidence from professional colleagues, not from traumatised patients.

• Why were there only 26 witnesses? Why not 200 or 2,000? I am sure Julie Bailey, the relatives' spokeswoman, could summon up an army of witnesses, patients and relatives, who suffered over the long years of the Mid Staffs trauma. Or maybe, their evidence, confused and emotional as it would be, would be dismissed as anecdotal" by the fine legal and medical minds of the GMC.

• Why does the GMC not punish managerial incompetence? Tom Kark claims that "all of the relevant doctors have been described as good or excellent clinicians in their own fields and there is no evidence whatever that they personally delivered poor care". But surely, their clinical competence is not at issue here. They were medical managers, and it is their lack of managerial skill and downright negligence that should be investigated. Even if you can't find sins of commission, they have clearly committed sins of omission.

• Tom Kark, in his judgment on the insufficiency of evidence says that "the case would have to be based on evidence of very poor management". There was plenty of evidence for that, I would, as a layman, have thought. I suppose it is the wrong kind of evidence, unfamiliar to the GMC in its day-to-day work of striking off individual doctors for fraud or whatever. Apparently in 2012, the GMC did review the duties of doctors in management positions. Not, apparently, enough of a review to cover the Mid Staffs case. Will the GMC launch another review to cover Mid Staffs-type scandals, and make its procedures fit for purpose?

• Does the GMC ignore past wrongdoing by doctors? Kark says "it is not impairment at the time of misconduct which the panel is looking for, it is impairment of the doctor's fitness to practice at the time of the hearing." Does that mean that the GMC cannot strike a doctor off if they keep their nose clean and if the hearing is delayed for long enough?

• Do Tom Kark and the GMC consider the hospital board's "systemic management problems" to be the sole cause of Mid Staffs, and all the doctors to be whiter than white? If so, they have lost all sense of reality.

It is not only the GMC that has shot itself in the foot with this crazy judgment. It is the whole NHS.

At this time, the program is limited to Homeschool Student IDs. Home educators, teachers & parents are not eligible to receive an ID card through this program.

The program is reserved for grades K-12 only.

If you made a mistake, you may re-submit the request for an ID card with revised information. We will do our best to send you a card using the last request for each student.

Want a new way to enjoy more fruit? Try this peach honey spread. Slice up 1 fresh peach or substitute canned peaches, but drain them first. In a bowl, combine peach slices, 2 tablespoons honey and 1/2 teaspoon cinnamon. Mash with a fork until the mixture is the consistency of chunky applesauce. Serve as a topping for pancakes or French toast, or serve it over roasted chicken or pork.

Mascara Monday: Too Faced Better Than False Lashes.

I was very intrigued by this new lash system from Too Faced called Better Than False Lashes. Of course, it promises big bold lashes, but in a different way as it uses actual lashes fibers in a multi-step system. I know there are a few fiber lash systems out there, and having never tried any makes this process new to me.

You get two tubes, which make up the 3 steps. One tube is the first step, called the Activating Base and Top Coat. The next tube is the Nylon Lash Fibers. You simply apply the base coat to the lashes, followed by the nylon fiber step, and then finish with another coat of the first product, which is now used as a top coat.

Basically, the first step is a mascara. Used alone, it gives nice length, separation and volume to the lashes. The next step, which I was nervous about because I wear contact lenses, is the fiber application. I was worried that I would get all these little fibers flying into my eyes, but I did not. The nylon fibers clung to the base coat and didn't budge. Phew!  I immediately noticed that I had a lot more lash to build upon, which was apparent when I applied the first product again, now the top coat. My lashes tripled in volume and even more length was added. I was so impressed!

For everyday, depending on the mascara, I use one to two coats. For an all out smokey eye look or night-time eye, I almost always use three coats of mascara and this system takes no more time than that. The difference is the end result when using Too Faced Better Than False Lashes because my lashes are the biggest and boldest that they have ever been without wearing actual falsies.

Discussion: How could the NHS better embrace research?

Clinical research drives improvements in healthcare and leads to the development of more effective treatments for patients. The government has said it wants to encourage health research, recognising that - at a time when demand on the NHS is high and finances are strained - being innovative can help improve productivity.

However data suggests that clinical research is not always embedded in NHS 'core business'. A lack of funding, time, expertise and resources are perceived to be the main barriers to clinical research by health professionals.

In our next online discussion we will ask if the NHS is really embracing research and whether it could it do this better. How can the barriers be overcome and how can trusts be encouraged to embed research into their core business? Is research seen as an extra burden rather than something that is integral to improving patient care, and should NHS leaders try to change this perception? How do we build a research active NHS? We will also explore how research can help the NHS meet the challenges of the future.

Getting Healthy with Nutrisystem: Week 9.

A funny thing happened on the way to the farm this week. I ran out of my Nutrisystem food. There was a little snafu with my order, and though all is well and I am totally back on my plan now, I definitely struggled a bit.

I honestly didn't think I was struggling though. I did what I always do when a problem arises; I came up with a solution. I thought about how I would transition off of Nurtisystem and that is what I did. I stuck to my eating schedule and substituted regular food from the grocery store. Well, food that I thought was comparable. I choose lower calorie, lower sugar, high protein and high fiber food. I ate at the same times that I normally eat my Nurtisystem food. I managed. Though I really have to admit that I missed the food, especially my favorite protein packed Fudge Graham Bars.

What I didn't manage to do was lose any weight. I guess it goes to show how powerful and how well Nutrisystem really works. In just five days of being off of Nutrisystem and eating similar foods, I gained 3 pounds, bringing my weight loss down to just 12 pounds. It was not what I was expecting.

'My job involves giving children a sense of control over their environment'

Council: Buckinghamshire county council

What is an educational psychologist? Educational psychologists advise local authorities in developing policies around education, carry out psychological assessments on children and train school staff in dealing with vulnerable children. We also have a role supporting families throughout Buckinghamshire..

Describe a typical day in your job: What I do is very interesting because it is so varied. It can involve seeing children, or working with local authority policy and practice at quite a senior level.

What kind of children do you work with? Children with no school place, or those with complex behavioural difficulties or significant mental health conditions. We also work with schools to produce materials for children who move home a lot, such as those whose parents are in the military. Military children who have lost a parent because of war or injury would be a priority group for us.

How do you help them? People have to adapt to new environments and psychology can help us understand what that means and how best to do that. That's why we developed "my passport" – a booklet that children take with them when they move schools. They can fill out information in it about themselves and their family, giving staff a better understanding of their background and history. The idea is to increase the child's sense of control over their environment so they settle in quicker.

What other interesting projects have you worked on? I've been talking to health professionals about the possible impact of returning veterans who have experienced trauma or post traumatic stress disorder and the possible impact that will have on their children.

What are the biggest challenges you face in your job? I work with some extremely vulnerable children and families, which makes demands on you personally. I think the variety of the job is an advantage – it makes it interesting – but it requires a very wide skill set. It's also keeping up to date with psychological literature; we are expected to do that. I have just finished a four year doctorate that I was doing concurrently with my job.

How do cuts and the changes to education the government is bringing in affect you? One of the ways we have reacted is to develop our services to make money. Schools that have been turned into academies have more control over their budgets and they can buy additional services from us now, such as training. So we've actually recruited additional psychologists who work in a different way.

How do you lead your team? We have 35 psychologists, some part-time some full-time, and it's very much a collaborative approach. The psychology of interaction is a crucial part of what I do, and giving advice to people isn't always the right thing to do. It is about working as part of a team as opposed to telling people what you think they ought to do.

What made you decide to be a psychologist? I was a teacher originally. I spent over 10 years working in a school with children with challenging behaviour and I became curious as to why they behaved the way they did and the best ways to help them. I decided to do a degree in psychology, and that led me to where I am now.

What made you decide to work in local government? The core of it for me is around public service ethos. Where I'm more comfortable is working with children and families who have the highest needs and who are the most vulnerable. And working for a local authority allows me to work in policy development as well at various levels.

What advice would you give to aspiring educational psychologists? Do a properly recognised psychology degree and get some experience working with children and young people and their families in whatever context. I think that's quite a good career route.

Who inspires you? Aneurin Bevan with the National Health Service – that ethos of helping other people but having organisations and structures that can do that.

This one is all about fall. It features bold colors for the lips, subtle smoky shades for the eyes, and beautiful glowy products for the cheeks.

My favorite products in the collection are for the face -- or cheeks in this case. They included the very popular Hibiscus Kiss Powder Blush Duo again. This is a beautiful powder bronzer in a neutral shade that can be worn by anyone. It is a light shade of bronzer, and looks especially great when blended with the cool coral blush. For my cheeks, though, I am loving the Powder Blush in Bad Girl Gone Good. It is very wearable copper peach shade, with a beautiful satin finish. It is the perfect color to transition skin from summer to fall. I use a bit of the Cream Colour Base in Diamonds, dabbed high on my cheekbones to finish the cheek look.

Of course, the star product of the collection is RiRi Woo, another relaunch. This time, it includes RiRi Woo Lipglass that totally completes the look. As I am not a huge fan of red on myself and prefer to wear a red gloss, this cool red cream shade is it for me.

The collection finishes with subtle smoke and drama for the eyes. Each palette contains four easy to wear colors in either light browns or light grays. They are both lighter than what I expected, so the emphasis is on the subtle when I say smoke. The drama comes from the Pointblack Liquilast Liner and the Extended Play Lash Gigablack Mascara.

Guardian unveils Healthcare Innovation Awards shortlist

A school, a charity, and a social enterprise are all in the running for the first Guardian Healthcare Innovation Awards.

The shortlist for the awards includes entries from outside the healthcare sector, as well as NHS trusts and clinical commissioning groups.

The awards, sponsored by GE, Unipart and 3M, are an opportunity to celebrate and share best practice across different areas of the healthcare sector – both inside and out of the NHS.

They aim to showcase ideas or services which significantly improve the quality or management of care for patients and share them with the rest of the sector.

The judges, including Dean Royles, chief executive of NHS Employers, Chris Hopson, chief executive of the Foundation Trust Network, and Dame Barbara Young, chief executive of Diabetes UK, looked at a number of entries in different categories including service delivery, technology, hospital admissions, leadership, partnership and workforce.

Here is the full list of organisations shortlisted this year. The winners will be announced at a ceremony on Thursday 24 October 2013 in London.

Five tips: how the private sector can drive change in the NHS

The healthcare system in the UK is facing a very serious crisis, with a number of chronic demand challenges and supply side issues putting the viability of the entire system at risk. The King's Fund estimates that, in the absence of a policy response, the growth in healthcare spend would require public sector debt to increase by up to 230% of GDP by 2060.

Despite this stark reality, the government is struggling to find a way of resolving the issue. More than 40 major healthcare reforms have been introduced since 2000, but their impact on controlling costs and improving productivity has been negligible at best – showing that pushing change from the top alone is proving ineffective.

The questions facing us now are fundamental and touch on the founding principles of the NHS: can the system ever be made sustainable? And is rationing care, compromising universal access, the only option?

We think not. Reducing costs without compromising quality and access to care is not impossible – but the path to creating a sustainable healthcare system needs to involve both private and public providers, with the private sector driving more of the change rather than merely reacting to government reforms.

Private sector participation in health delivery has been increasing in recent years, but still accounts for less than 10% of total spend. The government and the private sector are yet to develop ways of working together efficiently, and private providers continue to be viewed with suspicion in parts of the NHS. While this is true, the private sector has also been overly cautious and lacked the vision to influence government to implement change.

What can the private sector do to help the government drive change? Although there are no infallible recipes for success, there are a number of key principles private providers can follow to help the government make the NHS more sustainable:

• Be flexible and tailor solutions to fit specific local needs: one-size-fits-all, top-down approaches have proven to be unsuccessful time and again, with the National Programme for IT being a case in point
• Be in the driving seat of innovation: build capabilities to develop innovative, integrated care solutions rather than providing ingredients for the NHS
• Be willing to take more risks on outcomes
• Be realistically ambitious: start small, learn from mistakes and replicate success
• Be a partner, not a supplier: invest more time and effort in helping the government to transform the NHS, not just reactively, but responding to policy proposals.

Examples from around the world, where the private sector has been more assertive, show how effective a public-private partnership approach can be in reducing costs and achieving better outcomes. The Manises public-private partnership in Valencia, Spain, where a model of integrated care has been deployed is a good example – per capita funding is 25% lower than publicly run systems and clinical targets have been surpassed thanks to a combination of innovative approaches to care delivery (particularly keeping people out of hospital) and early interventions on prevention and diagnosis.

There are, of course, a number of principles that the government should also adhere to that will lead to innovative approaches to service delivery. For instance: investing in preventive care to avoid future costs; embedding technology into a locally integrated ecosystem to ensure patients are getting the right treatment, at the right time, in the right place; and, crucially, migrating to contracting on the basis of outcomes rather than inputs. These principles are easier to articulate than to implement, and the real challenge will be how to translate these objectives into action to deliver lasting change.

The journey will not be easy, but its success will be critical for the prosperity and wellbeing of the country. The time for providers to act is now.

Network of general practices aims to improve standards

Most people will have noticed that there is something up with the NHS and that Jeremy Hunt is no white knight, no matter how much he tells us he is. To expect GPs, facing a funding freeze despite burgeoning demand driven by demographic change and heightened consumer expectation, to come riding over the horizon may seem unlikely. However, that is what our new network of high-quality general practices, Quality Practice, is setting out to do.

Quality Practice is a national network of general practices that is designed to enhance standards of patient care and lift medical morale, as well as to strengthen practices by extending their core activities, share back-office services and increase efficiency, so that general practice will be able to deliver more for less, for everyone.

So, why form the Quality Practice network? Simply, because there seems to be no prospect of external investment in general practice. Quality Practice intends to bring new NHS and private income streams into practices. This may sound more like business than the caring profession but investment in primary care has been identified as a prerequisite for an effective healthcare system, and the NHS is not going to achieve the necessary shift of provision of more complex care into the community without it.

Sixty-five years on, Nye Bevan would readily recognise the general practice of today, and in that lies both its success and its weakness: personal, local and responsive on one side, but disparate and poor at working more corporately as a health community on the other. This builds in inefficiency and slows the adoption of new ways of working between practices, and, at least in part, explains the considerable variation in performance.

How can we achieve more consistently high standards? Albert, our social network platform, developed by Interact Intranet is our solution. Albert allows our member practices to work as one large virtual practice, pooling their talent, enhancing their practice and that of every member practice, with every good idea they share.

We also have a growing number of small federations, drawn together by geography, and in inner cities, the formation of "super practices" such as the Vitality partnership, which employs dozens of GPs. These might be seen as competition for the Quality Practice model but also clearly demonstrate that the need for change is not just being felt on the ground, but acted upon.

So how will Quality Practice differ? The Quality Practice model differentiates itself by scale: our ambition is to get to a total of around 500 member practices over the next four years, to both maximise economies in shared services and to provide local outlets for regional and national contracts. Ownership will also differ, with our member practices owning shares in the organisation.

They will have to have an outstanding commitment to continuous quality improvement that is essential for sustaining our relationship with NHS and other commissioners.

Setting up this sort of organisation at this time in the NHS's evolutionary stage means we have to contend with practices barely coping with everyday demands. But, working with our foundation practices in London, Manchester, Bristol and beyond, we are convinced that we have the model right. Funding is tight in practices, and so our financial model has to be solid. This was successfully tested in a public fundraising session through Crowd Cube recently, bringing in two more early investor practices and more than 40 private investors.

Greater consistency between general practices while simultaneously providing an extended choice of services for patients, as varied as chemotherapy and eyelid surgery, are essential traits of future NHS general practice. Achieving this is how Quality Practice will re-set general practice as the cornerstone of the NHS.
